One thing about this though, if we are adding additional spots, I think we need to address the fact that players can be left on IR once healthy till the end of the year with no cap hit.

This is a nice way to circumvent the salary cap, if you have a high priced under performing player. He gets hurt for a week, then I can move him to IR, collect that money back and just leave him there all year.

Just my opinion, but either LTIR players salaries should count against you cap, or you should have to remove those players within a certain amount of time of them being healthy. With one spot it wasn't a big deal, but when you talk 2 or 3, you can really influence your salary cap.

Right, but I just don't think real teams would keep a 4M guy out of the lineup to just not pay him, but maybe.

I think the proposal is that only one LTIR spot exist that allows you not to pay cap hit.  The 2 other IR spots , you still pay this cap hit but gives you an extra roster spot to work with.

It appears we have consensus support for this.  I'm adding 1 IR slot effective immediately.  Here's the official rule:

Injured Reserve
Each team will have one Injured Reserve slot.  Players on IR count against the cap, but do not count against the roster size, allowing for teams to add a replacement player via FA or call-up from minors while the injured player is out.  Any player missing games for any type of injury, whether they're day to day or out longterm, is eligible for IR.  Basically, if there's a red flag next to their name on Fantrax, they are eligible.  Once a player on IR is healthy and back to NHL action they may remain on IR until the end of the current season, at which point all IR players who are healthy will be added back to their teams 30 man roster
